In communities facing disparities in access to care, higher rates of chronic disease, or a lack of cultural fluency among healthcare providers, even a health check-up cannot be routine.
That’s why we provide compassionate, culturally responsive care in everything we do.
Services can include:
- Medical history review
Great care starts with a review of your medical history, including any past illnesses or surgeries, your family medical history and current medications. But Care, Where You Live goes further to discover the environmental factors that affect how you access care.
- Physical exam and lab tests
A routine exam may include checking blood pressure, heart rate, breathing, temperature, and weight. You may order laboratory tests to assess your health as well, including blood tests, urine tests, and imaging tests such as X-rays.
- Health risk assessments and lifestyle counseling
We assess your risk for various health conditions based on your lifestyle, medial history and other factors. With this assessment in hand, we can provide guidance and advice on how to maintain a healthy lifestyle, including recommendations for diet, exercise, stress management and even smoking cessation. - Vaccinations
Include Hepatitis B; Flu Shot; Traveler’s Medicine;
Tetanus and more. - Specialists referral
As needed, we provide referrals to specialists or other healthcare providers for further evaluation or treatment.
