Care Medical Practice Portal offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With the Care Medical Practice Portal, you can use the Internet to:

  • Request medical appointments.
  • View your health summary from the Care Medical Practice Portal electronic health record.
  • View test results.
  • Request prescription renewals.
  • Access trusted health information resources.

The Care Medical Practice Portal is a free service offered to our Family Members.

Patients who wish to participate will be issued a Care Medical Practice Portal activation code during their clinic visit. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during your next office visit.

Your test results are released to your Care Medical Practice Portal account after your physician has reviewed them. This is generally within 10-14 days.

 

Your provider is able to determine which types of test results are able to be accessed through the Care Medical Practice Portal. Further, tests of a very sensitive nature are not released to the Care Medical Practice Portal.

 

Your Care Medical Practice Portal information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

For your security, your access code expires after 60 days and is no longer valid after the first time you use it. If you still have problems, email us.

 

No, your access code is not your Care Medical Practice Portal ID or password. You will use this code only once to log into Care Medical Practice Portal for the first time. (The code will expire after you have used it or after 30 days). When you log into the Care Medical Practice Portal the first time, you will then be asked to create your own unique Care Medical Practice Portal ID and password.

  • While logged into AOL, click on the blue ‘e’ icon in the lower left-hand corner of the screen. It is located next to the Windows button (Windows 7 and Vista) or Start menu button (Windows XP, 2000, etc.). This will open a new Internet Explorer window outside of your AOL workspace.
  • Log into the Care Medical Practice Portal in the Internet Explorer window.

As of July 1, 2024, Primary Care Services at Care Medical Practice in Buffalo, NY will be in one central location at our Niagara Street location.
